TREATMENT AT MURACK LODGE
Initial Service Plan:
The Initial Service Plan will be developed with the resident, the resident's parents or guardian and the referral source, unless documented as inappropriate. The Initial Service Plan for each resident and family will comply with all of the following provisions:
a. Assess the resident's and family's strengths and weakness in the areas of education, economics, psychology, society and health as these relate to the resident's needs.
b. Identify plans for parent and child visitation.
c. Specific treatment goals to remedy the problems of the resident and family.
d. Set time frames for achieving goal and identify indicators of goal achievement.
e. Specify the persons responsible for coordinating and implementing the goals for the resident and family.
f. Specify staff techniques for achieving the resident's treatment goals.
g. Specify the projected length of stay and next placement.

14. Substance Abuse Prevention Program: Murack Lodge is licensed through the Michigan Department of Community Mental Health to provide a substance abuse prevention program to the residents. Murack Lodge holds weekly meetings facilitated by the Social Worker or Staff. Resident's participation in the program is mandatory.
15. Aftercare Services: An aftercare follow-up program will be designed for each resident who is leaving Murack Lodge and remaining in the Jackson vicinity. Aftercare will include scheduled meetings to be conducted both at Murack Lodge and at the home of the resident. The purpose of these meetings will be to further organize and maintain a weekly budget and the development of short as well as long term plans. Each resident will keep in touch with his advisor twice weekly to accomplish this goal. Scheduled meetings will take place with each resident's advisor for 3 months after release from the program. Visits will be made to the new residency of the youth by the advisor that was in charge of the release plan.
